Forms by Trust let you easily gather testimonials from your customers and clients. They can be Standard and Interactive, you can see their examples in this introduction article. The only difference between Standard and Interactive forms is how they look, so you can choose one that suits best your needs!

When you open the “Forms” tab for the first time, you see one default Standard form that you can start using immediately.

1. To create a new form, press the “New Form” button, then choose “Standard Form”.

2. Choose the “Texts” tab to define the main settings for your form.

The “General” section

Name a form

Give your form a private name that will be displayed only in your dashboard.

Collect a text testimonial | Collect a video testimonial | Collect external videos

Click on toggle buttons to specify which customer feedback types you want the form to include.

Set “Collect external videos” on to let your customers share their existing video testimonials from YouTube, Facebook, Instagram or Vimeo.

If you want to make a reviewer’s photo uploading necessary, turn the appropriate feature on. Without sharing a photo or logo, a person won’t be able to submit their testimonial.

When the feature is turned off, it’s up to the reviewer to decide whether to upload an image or proceed without it. Trust will replace it with a silhouette image later on.

E-Sign Contract

Switch the “E-Sign Contract” feature on if you want to e-sign GDPR compliant contracts with your reviewers and make it a part of the testimonial collection process.

❗️Please make sure that you have added necessary information to the “Contract Data” tab.

The “Header” section

Title | Logo

Give your form a public name that will be displayed in the header of your form.

Upload your business’ logo or choose one from the dropdown menu.

The “Testimonial” section

Label Testimonial Tab | Questions

Edit or translate the default text if needed. This text will only be shown if you choose a “Steps” form template (the “Styles” tab).

List all your questions you'd like to ask in the “Questions” field.

You can enable or disable the live testimonial preview in the form.

The “Star Rating” section

Collect a 5-star rating | Star rating

Turn on “Collect a 5-star rating” to let people give your product / service a rating.

Add a question for a rating field.

The “Video” section

Edit or translate the default text if needed. This text will only be shown if you choose a “Steps” form template (the “Styles” tab).

The “External videos” section

If your form allows to add video testimonials from social media and video sharing platforms (YouTube, Facebook, Instagram or Vimeo), you can specify which exactly videos you want to collect.

The “Public Data” section

Section headline | Your comments

Let people know that these fields collect public information that will be shown on your website later.

Change / translate the description or leave it empty if needed.

If you prefer to show customer data fields first before asking for a testimonial, you can add short greetings here, for example:

Define what public information you’d like the form to collect from your customers. The first field is always required (e.g. Name) while the second one is optional and can be removed from the form.

The “Private Data” section

– Collect private data, too

Turn this feature on of you want to collect other private information from your customers in addition to their email address. It will only be shown in your Trust dashboard.

You can also remove the “Company” field from the form by leaving it empty.

The “Consent checkbox” section

Collect reviewer’s consent | Consent text

You can add a checkbox field with a custom consent message to the form.

You can also add a link to your public privacy statement / data confidentiality agreement / any required legal texts here.

When this feature is on, a reviewer is required to tick a checkbox for a testimonial submission.

Later you can also find the date and time when you received that consent (edit the testimonial and go to the Private data” tab).

The “Social Media” section

Ask for Social Media Profile

Set “Ask for Social Media Profile” collect links to business websites or social media profiles of your reviewers. They will be displayed as a part of their testimonials on your website for more social proof. For example:

The “Footer and Button” section

Edit the text if you want to rename / translate buttons.

3. Go to the “Styles” tab to define the design of your form.

– Select the “Onpage” or “Steps” form template from the dropdown menu.

– You can choose whether the form shows a testimonial or public / private data fields first.

– Enable “Advanced Settings” to customize the form’s colors and font sizes.

– To reset the settings, click the circle button.

4. Choose the “Video Message” tab to upload or record your own video and personally say hello, thank customers and clients for their time, and give an incentive that motivates them to fill out the form.

It will pop up right after a reviewer opens a form, and disappear after being played, see the example below.

5. Open the “Guidelines” tab.

Turn on this feature to add any instructions that you consider necessary for your customers or clients.

Edit or translate a default text if needed.

Reviewers will see it on the right side of the testimonial request form.

6. Go to the “Thank You Page” tab.

– Add some text to show people your appreciation.

– Enable the “Rate on other services” feature to ask people for reviews on other platforms as well.

– Set the minimum star rating to prevent people from sharing negative reviews.

– Edit or translate default texts if needed.

– Turn on the necessary platforms and add links to your profiles there.

You can also add 1 custom platform that’s not listed above.

A reviewer will see profiles you added in form of buttons after their testimonial is submitted.

7. Press the “Save” button when you’re ready to finish.

8. You can add the form to your website or copy it’s URL to start collecting testimonials.

If you want to create a similar form based on an existing one, press the “Clone” button to have its copy, and now you can adjust the settings.
