How to set up an Interactive testimonial request form from scratch.

Forms by Trust let you easily gather testimonials from your customers and clients.

Interactive forms let you interview customers step by step.

They also include question and answer pairs that help you create engaging video surveys and text question & answer series that you can combine in one testimonial.

To create a new form, press the “Add New” button, then choose “Interactive form”.

The “General” tab

Choose the “General” tab to define the main settings for your form.

Name a form

Give your form a private name that will be displayed only in your dashboard.

Logo

Change your your business’ logo if needed by choosing one from the dropdown menu or uploading a new image.

Colors

Customize the form’s default colors.

Background color is a color of your form's background.

Primary Color is a color of a 'Start' and 'Next' buttons, and a progress bar.

Secondary Color is a color of a 'Back' button.

Edit or translate default texts if needed.

The “Welcome” tab

It lets you configure the welcome page of your Interactive form (the first page they see when they open the form).

Title | Subtitle

Write any intro text to welcome your reviewers and encourage them to leave you a testimonial. You can also add any links or format the text accordingly.

You'll always see a real-life preview on the right side of your screen.

Colors

Upload a high-resolution background image and change the default colors of title and subtitle texts.

Label

Edit or translate a default text if needed.

Welcome

Enable the “Show a video message” feature to upload or record your own video and personally say hello, thank customers and clients for their time, and give an incentive that motivates them to fill out the form.

It will pop up right after a reviewer opens a form.

The “Video Message” tab

Upload or record a welcome video message.

Once done, you'll see a video preview.

The “Testimonial” tab

Here you can set up configure the settings for text and video testimonial fields.

You can add multiple question and answer pairs or leave only 1 pair.

1. Press the “Edit” button to configure your first question and answer pair.

Question

1. Choose the question type of your first pair.

You can either ask a question in a text or record / upload a video question.

2. If you choose the “Text” option, add your question to the appropriate field.

3. If you choose the “Video” option, record or upload a video question.

You will see the video preview on the right side of the screen once it's fully uploaded.

Question subtitle

Add a question subtitle to offer reviewers choose a reply option.

Answer

1. Choose the answer type of your first pair.

If the “Both” option is chosen, then your reviewer will decide on an answer type (either write a text reply or record a video answer).

Text or Both answer type

Edit or translate the default placeholder text if needed.

Video or Both answer type

Edit or translate default texts for the video recorder if needed.

Show guidelines?

Turn on this feature to add any instructions that you consider necessary for your customers or clients regarding their text testimonial.

Edit or translate default texts if needed. You can also add any links or format the text accordingly.

These instructions will be shown for both “Text” and “Video” answer types.

2. Press the big + button to add more question and answer pairs.

The “AI Help” tab

Interactive forms have the AI testimonial generator.

This built-in AI writes an authentic testimonial text based on keywords that your customer has in mind about your product or service.

This feature aims to help your customers and clients provide you with a high-quality testimonial effortlessly, overcome the fear of a blank page and save their time.

It only works if the “Text” testimonial is enabled.

1. Turn on the “Enable AI based Text Generation” option to add the AI testimonial generator to your form.

2. Choose your form's language.

Now only 3 languages are available, but you can contact us to get your native language added!

3. Choose your business type.

Also, contact us if you don't see your business type in the list.

4. Add your business name and its short description.

5. Edit or translate default texts if needed in the “Text configurations” section.

The “Tooltip” field lets you add custom instructions to help customers generate the best testimonials.

The “Stars” tab

1. Turn on “Ask for a Rating” to let people give your product / service a rating.

2. Set up colors for empty and filled stars.

The “Personal” tab

Configure which information you want to collect from your reviewers.

1. Give the “Personal information” page a title. Edit or translate default texts if needed.

2. Leave the “Placeholder Title Company” field empty if you want to remove it from the form.

3. Edit or translate default texts for the image uploader if needed.

4. Edit or translate default texts for the image cropper if needed.

The “Socials” tab

Set “Ask for Social Links” on to collect links to business websites or social media profiles of your reviewers. They will be displayed as a part of their testimonials on your website for more social proof. For example:

The “Send” tab

Set up the confirmation page of your Interactive form.

1. Give the page a title. Edit or translate the default text if needed.

2. You can add a checkbox field with a custom consent message to the form.

You can also add a link to your public privacy statement / data confidentiality agreement / any required legal texts here.

When this feature is on, a reviewer is required to tick a checkbox for a testimonial submission.

Later you can also find the date and time when you received that consent (edit the testimonial and go to the Private data” tab).

3. Turn “Confirm Email” on to send a reviewer a confirmation email and verify their email address.

You can also change the color of the email confirmation box.

4. Edit or translate default texts for “Cancel my review” and “Send” buttons if needed.

5. Set up the page which the reviewer will see after they submit their testimonial.

If you want to show a built-in Thank You page by Trust, choose the “Show Thank You page” option.

You can also redirect reviewers to any other page by copying and pasting its URL.

The “Thank You” tab

Set up the “Thank You” page for your Interactive form.

1. Give the page a title and add some text to show people your appreciation.

2. Enable the “Rate on other services” feature to ask people for reviews on other platforms as well.

Set the minimum star rating to prevent people from sharing negative reviews.

3. Edit or translate default texts if needed.

4. Turn on the necessary platforms and add links to your profiles there.

You can also add 1 custom platform that’s not listed above.

The “Email” tab

Configure the page that your reviewers will see after they confirm their email address.

1. Give the page a title, and edit or translate default texts if needed.

2. Press the “Save” button when you’re ready to finish.

3. You can add the form to your website or copy it’s URL to start collecting testimonials.

4. If you want to create a similar form based on an existing one, press the “Clone” button to have its copy, and now you can adjust the settings.
