Create an Interactive form
Forms by Trust lets you quickly gather testimonials from your customers and clients.
Interactive forms let you interview customers step by step.
They also include question-and-answer pairs that help you create engaging video surveys and text question-and-answer series that you can combine into one testimonial.
Simply follow these steps to create an Interactive form.
1. Navigate to the Forms menu.
2. Click the Add new button and select Interactive form.
3. Choose a template or start from scratch by setting up your questions.
4. Edit and customize the interactive form fields on each tab.

5. Click the Save form button to create the interactive form.
6. Finally, you can add the form to your website or copy its URL to start collecting testimonials.

Here are your quick options to share a testimonial request form:
Option 1: Copy link button - share a direct link with your clients.
Option 2: Embed to website (e.g., Inline or As pop-up) - embed the Smart form to your website
In addition, if you want to create a similar form based on an existing form, click the vertical ellipsis icon and select “Clone” to create a duplicate, then customize the form details accordingly.
